Case Management

The Case Manager has a pivotal role in assisting injured and ill clients return to health and fitness. Clients will have a range of needs, according to their individual circumstances and type of injury.

The goal of the Case Manager is to assist the injured person to their pre-accident status and if possible return to employment through the co-ordination of appropriate services. The client remains the Case Manager’s primary focus.

The process begins with an holistic assessment in conjunction with the injured person (and including family if appropriate). This assessment encompasses physical and bio psychosocial factors, taking into account the client’s individual circumstances, and identifying any barriers to recovery.  If the injured person has serious injuries and/or complex needs this assessment will take place in the client’s home. 

A written report (Initial Needs Assessment) will cover all relevant medical details and include the client’s social and housing situation as well as physical, psychological, vocational and financial aspects. The rehabilitation plan is formulated in conjunction with the client with recommendations being made along with the rationale and goal (and cost) for each.
We work in collaboration with all parties and therefore the Case Manager is available for further discussion with all parties regarding the rehabilitation plan for the duration of the case management intervention.

The benefits and effectiveness of the interventions are monitored and evaluated by the Case Manager in conjunction with the client throughout the life of the case. The rehabilitation plan may need to be adapted according to the client’s responses to the interventions and or any change in the client’s circumstances.
